Clinical Case Analysis: Comparison of OdneClean Advanced Irrigation and Standard Needle Irrigation (SNI) followed by Endoactivator

Overview

This clinical case analysis evaluates the impact of OdneClean advanced irrigation compared to standard needle irrigation (SNI) across 100 clinical cases. The investigation assessed treatment time, post-operative pain, and the ability to clean and obturate complex root canal anatomies.

The study included blinded radiographic analysis and a validated scoring system for complex anatomy identification.

Findings – Clinical Case Analysis

  • OdneClean (Group A) shows filling of more complex / hidden root canal anatomy than SNI (Group B) indicating the OdneClean improves cleaning of complex anatomies (e.g. lateral canals / apical deltas)
  • Both groups show same number of apical puffs indicating that apical patency and proper obturation was achieved in both groups
  • No significant increase in treatment time using OdneClean.
  • No significant difference in post-op pain. 2 out of 50 called in after OdneClean. 3 out of 50 after SNI.

Method

  • 50 clinical cases with SNI (standard needle irrigation) protocol followed by NaOCl activation with Endoactivator
  • 50 clinical cases with OdneClean advanced irrigation protocol (3 cavitation cycles of OdneClean using sterile water followed by final NaOCl rinse)
  • Treatment time was tracked (from entry to temp restoration) / post-op pain was tracked (patient calling-in)
  • Blinded analysis of x-rays to identify obturation of complex anatomies performed by one endodontist (Dr. Cecilia Leopoldino) and a scientist (Dr. Else Ellermann)
  • Scoring system discussed and validated with 2 external Endodontists

Complex Anatomy Scoring – Scoring System

complex anatomy

Group A (OdneClean):

93 (50 teeth) | Average per tooth: 1.86 ± 3.01

  • Molars: 58 (34 teeth) | Average per tooth: 1.71 ± 2.96
  • Premolars: 32 (13 teeth) | Average per tooth: 2.46 ± 3.43
  • Anteriors: 3 (3 teeth) | Average per tooth: 1 ± 1.73

Group B (SNI/Endoactivator):

39 (50 teeth) | Average per tooth: 0.78 ± 2.17

  • Molars: 32 (39 teeth) | Average per tooth: 0.82 ± 2.96
  • Premolars: 7 (10 teeth) | Average per tooth: 0.7 ± 3.43
  • Anteriors: 0 (2 teeth) | Average per tooth: 0 ± 0

These findings reflect a substantially higher degree of complex anatomy obturation in OdneClean cases compared to SNI.

Treatment Time Comparison

Treatment time

  • Group A overall: on average 37.12 ± 12.19 min/case
  • Group B overall: on average 32.68 ± 9.59 min/case

Per case type

Group A (OdneClean):

  • Necrotic: on average 37.82 ± 12.25 min/case
  • IP: on average 39.63 ± 14.35 min/case
  • Retreat: on average 36.13 ± 12.48 min/case
  • Vital: on average 32.25 ± 7.89 min/case

Group B (SNI/Endoactivator):

  • Necrotic: on average 32.5 ± 10.63 min/case
  • IP: on average 32.31 ± 8.25 min/case
  • Retreat: on average 33.6 ± 9.73 min/case
  • Vital: 33 min/case

Summary

Across 100 clinical cases, OdneClean demonstrated:

  • Significant improvements in the cleaning and obturation of complex canal anatomies
  • Equivalent achievement of apical patency and obturation quality
  • Comparable treatment times relative to SNI
  • No increase in patient-reported post-operative discomfort

These findings support OdneClean as an effective advanced irrigation modality capable of improving anatomical cleaning without increasing clinical burden.
